CASE MANAGEMENT - HIV/AIDS
Provides basic case management at no charge for individuals who are diagnosed as HIV positive. Staff works closely with clients to evaluate their needs and to develop a comprehensive care plan to ensure all needs are being met. Staff works in collaboration with other healthcare professionals throughout the community and links clients to appropriate health and support services, ensures the delivery of services, monitors clients' progress, and re-evaluates the care plan as needed.
